1052 Intussusception of the Appendix in a Young Adult with Cystic Fibrosis: An Important Differential Diagnosis of Abdominal Pain in Cystic Fibrosis Patients?

Abstract Background Cystic fibrosis (CF) is commonly associated with gastrointestinal manifestations from infancy to adulthood. Intussusception in the paediatric CF population widely reported, whereas adult, distal intestinal obstruction syndrome (DIOS) common affecting 20%, of which, intussusception rare and appendiceal rarer, 1%. Case-Description A 20-year-old male presented 3 days right iliac fossa pain diarrhoea. On admission, he was hypotensive afebrile. Clinical examination revealed tenderness flank a palpable mass fossa. Serum investigations showed mildly raised inflammatory markers. Contrast CT abdomen-pelvis confirmed appendix into ascending colon. Histological analysis further demonstrated intussusception, intraluminal mucinous material. Fascinatingly, no transmural inflammation present. The patient underwent laparotomy-open appendicectomy. Interestingly, intraoperative findings resolved, longer requiring surgical reduction. Patient postoperative recovery complicated by pneumonia for which transferred specialist respiratory care thereafter, discharged. Discussion Literature review highlighted paucity data, 10 reported cases adult patients. we report had reduced time operation. This in-keeping previous case reports transient spontaneously resolved. Conclusion From this presentation, have learnt it imperative carry high index suspicion patients where acute appendicitis seen less often differential diagnoses like DIOS should be considered.
